An Accountable Care Organization (ACO) is a group of healthcare professionals working to provide Medicare patients with a coordinated care solution. Value-Based Care (VBC), used as the model of care within an ACO, is especially relevant when managing chronic illnesses, where proactively monitoring and treating patients generates better outcomes.

There is an intricate daily dance behind the scenes within the South Carolina House Calls (SCHC) Center for Telehealth. The Center comprises multiple disciplines, working together to provide in-home and telehealth patient care to South Carolina residents.

While writing articles about South Carolina House Calls (SCHC), I have had the privilege to interview many practitioners. With continued growth in call volume, it became imperative to find a solution to combine the latest technology and features to improve the customer experience.

With a broad array of commonly prescribed medications, action kits are now carried by South Carolina & Georgia House Calls Nurse Practitioners (SC & Ga HC). This proactive step is similar to a clinical visit where a doctor prescribes a medication and then gives the patient a starter supply until the prescription can be filled.
