Doctor

At Your Health, patients will have a doctor as part of their care team. The doctor's job is to assist them in making medical decisions based on their diagnosis and help them improve their overall health.

Nurse Practitioner or Physicians Assistant

At Your Heath, patients will have a physician assistant or nurse practitioner as their provider. As NPs & PAs, they are part of the care team that provides medical support for patient's health needs and goals of care.

Pharmacist

At Your Health, patients have access to a pharmacist who is a part of the care team. Your Health understands that medication problems are one of the main reasons people end up in the ER or the hospital.
