One of the exciting technologies utilized by SC House Calls (SCHC) in 2020 to improve patient Care was Remote Patient Monitoring (RPM). The technology, which allows healthcare professionals to monitor a patient's vitals remotely. This data has proven invaluable for people suffering from chronic conditions like Diabetes, Chronic Obstructive Pulmonary Disease (COPD), or Chronic Heart Disease (CHF). 

The RPM technology measures the oxygen level, weight, blood pressure, and pulse. It transmits those readings to a central location where medical professionals monitor and watch for changes. The monitoring can be a lifesaver for patients when changes occur because of the quicker response times of the SCHC Providers, which can save lives.
 Since launching the program in October of 2019, SCHC has served thousands of patients and continues to expand the program. SCHC is continuously looking for new and innovative ways to help patients live their healthiest life. The SCHC model of Patient-Centered Care seeks to provide a proactive approach with the convenience and accessibility of healthcare as a primary focus.  

SC House Calls is a network of nearly 300 medical professionals providing In-home medical house calls and Telehealth visits to Private Residences, Assisted Living Communities, and Skilled Nursing Facilities. SC House Calls serves all 46 Counties of South Carolina.
